"The correct response to failure is learning, not prevention." The conventional response to failure is to analyse root cause and introduce a control to prevent it recurring (and often to penalise or punish those involved). The global Safety-II movement in the OSH sector has transformed thinking about such responses, realigning us to prioritise improving future success over preventing future failure.Networking & Social Function
